This verse doesn't sugarcoat reality. It acknowledges that your body will fail you. Your emotions will fail you. Your physical health, your mental stamina, your emotional resilience all have limits. These aren't hypothetical possibilities. The psalmist says "may fail" as in when they fail, not if they fail. Failure is part of being human.
But here's the pivot that changes everything: "but God." Those two words are where hope enters the picture. Yes, your flesh fails. Yes, your heart gives out. But God is the strength of your heart. Not was. Not will be someday. Is. Right now, in this moment, when everything else is crumbling, God remains your strength. He is constant when everything else is variable.
The word "portion" here is significant. In ancient Israel, your portion was your inheritance, your share, your sustenance. The psalmist is saying that God Himself is what sustains him.
